Home Cancer Care Platform Reimagine Care is expanding its partnership with Houston-based Memorial Hermann Health System beyond its flagship hospital, Memorial Hermann-Texas Medical Center, to provide digital, virtual and at-home care for cancer patients.
Reimagine Care partners with providers and health plans to coordinate home care to avoid unnecessary hospitalizations. Reimagine clinicians can remotely monitor patients, collaborate with oncologists to determine if patients need additional services, and send home care workers to help.
Memorial Hermann and Reimagine formed a partnership last year, announcing news of Memorial Hermann’s investment in Reimagine Care.
The partnership will provide cancer patients with 24/7 access to a team of expert oncology caregivers. Patients can connect with team members via SMS text, video and phone to receive oncology-specific educational content, address concerns about symptoms and receive at-home care if needed.
“We are excited about our expanding partnership with Memorial Hermann and what it means for the future of patient care. Our deeper collaboration will enable us to combine Memorial Hermann’s extensive resources with our innovative solutions to make a real difference in the way patients receive cancer treatment.” Dan NardiThe CEO of Reimagin Care said: MobiHealthNews By email.
“This is all about making care more personal, efficient and effective. We believe this expansion will not only improve the quality of care we provide, but also lead to meaningful improvements in patient outcomes. This is a major step forward, and we look forward to the positive changes it will bring.”
